Nursing care in the home
Our registered nurses and licensed practical nurses visit on a schedule set by the physician’s orders and the care plan. Common reasons families call us:
- Wound and pressure sore care, including post-surgical dressings
- Medication management, setup, administration, and sorting out the confusion when someone leaves hospital with eight new prescriptions
- Diabetes care, blood sugar monitoring and insulin teaching
- Catheter, ostomy and feeding tube care
- Injections and IV therapy as ordered
- Vital signs and chronic disease monitoring for heart failure, COPD, kidney disease and hypertension
- Post-hospital and post-surgical follow-up to reduce readmission
- Teaching the family what to watch for and what to do at 2am
Therapy at home
Where it is ordered, we arrange therapy in the home so there is no travel involved:
- Physical therapy, strength, balance, walking and fall prevention
- Occupational therapy, dressing, bathing, kitchen safety and adapting the home
- Speech therapy, speech, swallowing and communication after a stroke
Do you need a doctor’s order?
For skilled nursing and therapy, yes. These services are ordered by a physician and delivered under a plan of care. If your loved one is being discharged from hospital or rehab, ask the discharge planner to refer to Millennium Home Health Care, or give us the hospital’s number and we will make the call ourselves.
Personal care and companion care do not require a doctor’s order. If you are not sure which one your situation calls for, phone us and describe it. That conversation is free and usually takes ten minutes.
