What the CCC Plus Waiver is
CCC Plus Waiver stands for Commonwealth Coordinated Care Plus Waiver. It is a home and community based Medicaid waiver, part of Virginia’s Cardinal Care program, that pays for services allowing someone to stay at home instead of moving into a nursing home.
Who it is for
- Older adults who need help with daily activities
- People with physical disabilities
- People managing a chronic illness or a serious medical condition
The common thread is a level of need that would otherwise require nursing home care.
What it pays for
- Personal care, delivered by an agency or through a consumer-directed attendant
- Respite care, a break for a family caregiver
- Other home and community based services, depending on the person’s plan of care
How to find out if your family member qualifies
- Confirm Medicaid eligibility, or apply if they do not have it yet.
- Request a screening through the local Department of Social Services.
- A nurse and a social worker assess daily living needs.
- If approved, the person chooses a health plan and a provider.
Timeline
Screening and approval usually take several weeks. Starting early is useful for any family, especially after a hospital stay when needs change quickly.
